  Criteria in the environment category concern resident comfort and safety, as well as, facility housekeeping, maintenance and sanitation. A facility must provide a safe, functional, sanitary and comfortable environment for the residents, staff and the public.

    The following federal regulation revisions and F numbers apply to surveys started on or after November 28, 2017.

  1. The facility provides residents with the right to retain and use personal possessions, including furnishings, and clothing, as space permits, unless to do so would infringe upon the rights or health and safety of other residents. (42CFR 483.10(e)(2) F 557 )
  2. The facility provides a safe, clean, comfortable, and homelike environment, allowing residents to use their personal belongings to the extent possible by; Ensuring that the physical layout of the facility maximizes resident independence and does not pose a safety risk; Providing housekeeping and maintenance services necessary to maintain a sanitary, orderly, and comfortable interior; Maintaining clean bed and bath linens that are in good condition; And providing private closet space in each resident room, adequate and comfortable lighting levels in all areas, comfortable and safe temperature levels and the maintenance of comfortable sound levels.   3. The facility ensures that the resident environment remains as free of accident hazards as is possible and each resident receives adequate supervision and assistance devices to prevent accidents. (42CFR 483.25(d)(1)(2) F 689 )
  4. The facility attempts to use appropriate alternatives prior to installing a side or bed rail. If bed or side rails are used, the facility ensures correct installation, use, and maintenance of bed rails, including but not limited to the following elements; Assessing the resident for risk of entrapment from bed rails prior to installation; Reviewing the risks and benefits of bed rails with the resident or resident representative and obtaining informed consent prior to installation; Ensuring that the bed’s dimensions are appropriate for the resident’s size and weight; And following the manufacturers’ recommendations and specifications for installing and maintaining bed rails.   5. The facility is equipped to allow residents to call for staff assistance through a communication system which relays the call from resident rooms, toilet and bathing facilities directly to a staff member or to a centralized staff work area (42 CFR 483.90(g)(1)(2) F 919 )
  6. The facility provides a safe, functional, sanitary and comfortable environment for residents, staff and the public. (42 CFR 483.90 F 921 )
  7. The facility maintains an effective pest control program so that the facility is free of pests and rodents. (42 CFR 483.90(i)(4) F 925 )

    The following federal regulation revisions and F numbers apply to previous surveys before November 28, 2017.

  1. The facility provides a safe, clean, comfortable and homelike environment and allows residents to use his or her personal belongings to the extent possible. (42 CFR 483.15(h)(1) F 252 )
  2. The facility provides housekeeping and maintenance services necessary to maintain a sanitary, orderly, and comfortable interior. (42 CFR 483.15(h)(2) F 253 )
  3. The facility provides residents with clean bed and bath linens that are in good condition. (42 CFR 483.15(h)(3) F 254 )
  4. The facility ensures that the resident environment remains as free of accident hazards as is possible; and each resident receives adequate supervision and assistance devices to prevent accidents. (42 CFR 483.25(h) F 323 )
  5. The facility ensures that the nurses' station is equipped to receive resident call through a communication system from resident rooms, toilet and bathing facilities. (42 CFR 483.70(f) F 463 )
